Thank you for making an appointment with Ozaukee Acupuncture and Herbal Services . Your first appointment will usually be 60 to 90 minutes. If you would like, you may fill out the Health History Form at home or at work by downloading and printing out the form. You will need Adobe Acrobat Reader(free) to access and print the file. Otherwise, please come to our office 15 minutes early to fill out the forms. If you need to cancel, please try to give us a 24 hours notice or you may be charged a $25.00 fee.
